The Virtual Ward
The Virtual Ward is a consultant led service for elderly care and to help older people stay at home instead of being admitted to hospital.
Based at St Lukes integrated intermediate care hub, it is run by health and social care staff who know what community services are available across Bradford and what their capacity is.
Patients can be referred by their GP, staff in the community and other health and social care professionals into intermediate care via the virtual ward. This could consist of visits from health professionals such as nurses and physiotherapists.
Intermediate care wards (Community Beds)
Community beds are for people of any age but usually older people. They are used for people who are too poorly and cannot be supported safely at home or people who are being discharged from hospital but they are still not well enough to go home.

Ward F5 St Luke’s
Westbourne Green Community Hospital
Westwood Park Diagnostic Treatment Centre.
Visiting Hours are 12.00 – 19.00
