These services include:

• The Virtual Ward
• Intermediate Care Wards
• Home From Hospital
• Bradford Enablement Support Team (Best)
• Community Matrons
• Case Managers
• Complex Care Team

These services  are not always known about or used by some members of our community.

The Engaging People Team will be running projects across the Bradford District and want to find out how much you know about these services, what you think about these services and what more could be done to help you use these services.
